RN Case Manager, FT Days

Schedule: Full - Time Regular

  Job Summary:  
The RN Case Manager assesses the social, psychological, cultural, environmental and financial situations concerning the patient and family. He/she is a patient/family advocate, promoting rights and dignity and striving to involve the patient/family in aspects of care as appropriate. The RN Case Manager also facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care for the populations listed below. Accountable for the care of patients assigned to them in accordance with the Nurse Practice Act and Policies and Procedures of BSA.

  Education/Licensures/Certifications:
  Current License as RN in State of Texas and Basic Life Support Certification.

  Qualifications/Knowledge/Skills/Abilities:
  Minimum of 2 years of experience as an RN in Med Surg. (Case Management or Utilization Review experience is a plus)
  Must exhibit desire to exemplify the BSA culture, including the BSA Mission, Vision and the BSA Way
  Good interpersonal and customer service skills    
  Good organizational and follow-through skills
  Ability to multi-task constantly and tolerate interruptions
  Position requires fluency in English language to function in job environment appropriately
