Physical Therapist

Shift: Day
Schedule: PRN Relief

  Job Summary:
Provision and documentation of examinations, evaluations, establish functional PT diagnosis, prognoses and plans of care including formulation of treatment goals, providing physical therapy intervention to patients, measuring and reporting outcomes and formulating and carrying out discharge plans. Supervision of licensed and non-licensed assigned staff or students. Interact and practice in collaboration with other professionals. Individuals must comply with the Texas Physical Therapy Practice Acts and Rules.

  License/Certification/Education: 
  PT licensed in the state of Texas
  CPR certified from the American Heart Association or the American Red Cross

  Qualifications/Knowledge/Skills/Abilities: 
  Physical Therapy experience in an acute care hospital and/or rehabilitation facility preferred
  Ability to effectively function as part of a multi-disciplinary team
  Ability to supervisor, delegate and communicate with multiple team members
  Top notch organizational and follow-through skills
  Excellent communication skills (written and verbal)
  Computer usage required EMR and Soarian documentation
  Compassionate and committed to providing excellent customer service
